Certificate 072/o issued by a specialist physician or therapist for a patient to visit a health resort with subsequent recovery. Form o72/o for a health resort is an integral document of the health resort institution, which gives the right to receive a voucher. The data specified in the health resort card are confidential and are not subject to disclosure.
Medical certificate form 072/o is issued in accordance with order No. 110 of the Legislation of Ukraine, on receiving a voucher to health resorts at a reduced price or free of charge for certain categories of persons.
How to get a health resort card?
Form 72 for a sanatorium is necessary to restrict access to sanatoriums and health resorts for people who are contraindicated for specialized sanatorium procedures. In addition, certificate 072/o will adjust the treatment itself, adequately building treatment and preventive procedures.
Medical certificate form 072/o does not replace a spa card, the most detailed and comprehensive medical document used in sanatoriums for each individual patient.
To obtain form 072/o, you must visit the following specialists:
- General practitioner.
- Narrowly focused specialist.
- Gynecologist (for women).
- Psychiatrist-narcologist (if necessary).
In addition to doctors, the following laboratory tests must be carried out:
- Complete blood count.
- General urine analysis.
- Blood sugar test.
- Fluorography.
- X-ray.
- Ultrasound examination.
- Echocardiogram.
- And others as needed.
If the patient has undergone the above-described examinations no earlier than a year ago, then there is no need to repeat them; it is enough to provide the results obtained.
After passing all the examinations, the attending physician or the head physician of the medical institution makes a conclusion, based on the results of which a certificate 072/o is issued and the profile of the sanatorium-preventorium is selected.
The certificate of form 072/o should be processed no later than a month before departure, since its processing will take some time. It is valid for 1 year from the date of receipt.
