- medical record form (sold in our office);
- results of bacteriological cultures (we do them if there are none);
- fluorography results (for this year);
- passport.
How much does a medical examination for a health certificate cost:
- Complete initial medical examination with tests – 900 UAH
- Secondary examination with partial analysis – 700 UAH
- Secondary examination without tests – 400 UAH
- Health book form – 50 UAH

A personal medical book is a special medical form containing special columns where the conclusions and opinions of doctors, laboratory data and admission to work, if the state of health allows, are recorded. It is created for the purpose of identifying and preventing infectious diseases based on the legislation of Ukraine.
A sanitary medical book is widely in demand at enterprises specializing in the food industry, public catering, trade, production of medicines and further work with them, medical and cosmetic services, public services, education and training of children and at other enterprises where a sanitary book is necessary.
The list of doctors who must be visited is established by the legislation of Ukraine and depends on the type of activity of a person. There is a primary (once a year) and secondary (after six months) medical examination. In the first case, the examination includes all the necessary tests (bacterial culture of flora for staphylococcus and intestinal group, blood and smears). During the secondary examination, not all tests are taken, since bacteriological cultures are valid for a whole year. A medical examination must be taken every 6 months and a health certificate must be renewed every 6 months.
