What is Form 100-2/o?
A medical certificate of psychiatric examination (form 100-2/o) is an official medical document that is issued to a citizen after undergoing a psychiatric examination by a psychiatrist. The type of examination directly depends on the purpose of the person being examined (for example, employment) and accurately establishes the absence or presence of psychiatric deviations (contraindications) for performing work duties or performing other actions.
There are several types of examinations:
- preliminary (before starting work);
- periodic (annual);
- extraordinary (if an employee shows signs of mental problems).
What documents are the basis?
According to the Cabinet of Ministers’ Resolution and the Ministry of Health’s Order, psychiatric examinations are mandatory for workers of certain specialties and people who need a medical certificate, for example, to obtain a permit to own a weapon. The regulations change the procedure for carrying out drug addiction and psychiatric examinations. They are:
- Resolution No. 57 of May 10, 2022 (“On approval of the list of medical psychiatric contraindications for the performance of certain types of activities (work, professions, services) that may pose an immediate danger to a person or others”).
- Order No. 651 of April 18, 2022 (“On approval of the Procedure for conducting preliminary, periodic and extraordinary psychiatric examinations, including for the use of psychoactive substances”).
With the entry into force of these regulations, the procedure for conducting mandatory narcological (form 140/o) and psychiatric (form 122/o) examinations has ceased to be in effect. In this regard, medical certificate 100-2/o completely replaces psychiatrist and narcologist certificates.
Who needs to undergo psychiatric examinations?
Medical certificate 100-2/o is required:
- pharmacists, employees of pharmaceutical companies;
- medical workers of medical institutions;
- employees of educational institutions, children’s institutions (children’s homes, for health and recreation, preschools);
- workers in certain industries where ethyl alcohol is used (alcoholic beverages, wine production, perfumery and cosmetics, dental products);
- drivers of vehicles of all categories (A1, A, B1, B, BE, C1, C, C1E, CE) and tractors;
- employees performing high-risk work, including high-altitude work, on mechanical equipment related to timber harvesting (cutting, rafting, transportation, processing).
How is a psychiatric examination performed?
The appointment begins with a visit to a qualified psychiatrist, who talks to the patient, collects anamnesis (illness, life), and sends for tests (blood, urine).
Laboratory tests include:
- detection of carbohydrate-deficient transferrin in the blood using capillary electrophoresis;
- detection of psychotropic and narcotic substances in urine (opioids, cannabinoids, sedatives, hypnotics, cocaine, hallucinogens, psychostimulants).
Based on the results of the examination and the tests received, a medical certificate of psychiatric examination in form 100-2/o is issued.
