According to the new requirements, the mandatory medical examination of employees is issued using form 025-9/o — “Certificate of completion of mandatory medical examination of an employee”.
This certificate has replaced previously used documents (for example, those issued under “Order 246 / occupational medical examination”) and is currently the official confirmation that the employee has passed the medical examination.
Certificate 025-9/o contains all the main information about the patient:
- name of the clinic that issued the document;
- surnames and positions, signatures and seals of the medical staff of the institution that conducted the employment medical examination;
- full name, date of birth, residential address of the patient;
- reason for issuing the certificate and the name of the organization where it will be submitted.
The medical examination is conducted by a certified medical commission in accordance with the instructions for form 025-9/o.
Who needs this examination?
The medical examination is mandatory for employees whose type of activity falls under the categories specified in the relevant regulations (work with harmful/dangerous conditions, work involving contact with people, specific professional groups, etc.).
