A health certificate (medical book) is mandatory for specialists in the following fields:
- public catering;
- trade;
- food industry;
- when working with water supply structures;
- in the field of transportation, storage and sale of medicinal products;
- in preschool institutions, educational institutions;
- in the field of providing household and public utilities;
- in medical and cosmetic services for citizens;
- in the field of production and manufacturing,
- other industries that require workers to undergo preventive medical examinations in order to continue working.
There are two types of sanitary book registration:
- initial examination (issued for the first time);
- secondary examination (extension of the initial examination, renewal once a year or every six months, depending on the requirements of the position held).
You can make a health certificate in our medical center. To do this, you must present the following documents:
- one 3×4 photo;
- the result of a completed fluorography (a copy with a triangular seal and a validity period of no more than one year);
- passport;
- tests.
The cost of a health certificate includes the costs of:
- examination by all specialized doctors;
- tests;
Additional charges:
- form;
- deduction of the minimum contribution to the SES.
The completed medical examination is certified by the signature of the head physician and the seal of the medical institution. The new sample health record has a separate page intended for printing with the inscription “fit for work” indicating the validity period of the completed medical examination.
